Healthcare Power of Attorney
Purpose: To designate an agent to make healthcare decisions. Legal Significance: Allows a trusted individual to make decisions when one is unable to do so themselves.
Medical Orders for Life-Sustaining Treatment (MOLST)
Purpose: To specify preferences for life-sustaining treatment. Legal Significance: Helps ensure that a patient's treatment preferences are followed by all healthcare providers.
Healthcare Proxy
Purpose: To appoint another person to make healthcare decisions if one becomes unable to do so. Legal Significance: The appointed proxy has decision-making authority which is recognized by medical institutions.
Out-of-Hospital Do Not Resuscitate (OOH-DNR) Order
Purpose: To indicate that a person does not want resuscitative measures if they are found outside of a hospital setting. Legal Significance: First responders and out-of-hospital medical personnel must comply with this order.
Emergency Medical Services (EMS) Do Not Attempt Resuscitation (DNAR) Order

Durable Power of Attorney for Healthcare

Five Wishes Document
Purpose: To communicate personal, emotional, and spiritual needs as well as medical wishes. Legal Significance: Recognized in many states as a binding legal document for healthcare decisions.
Organ Donation Directive
Purpose: To express one's wishes regarding the donation of organs and tissues after death. Legal Significance: Guides medical personnel and family in carrying out the organ donation process according to the individual's wishes.
