Living Will
A living will describes your preferences for medical treatment in situations where you are unable to make decisions for yourself. It becomes effective only under specific, typically end-of-life conditions.
Health Care Power of Attorney (HCPOA)
A Health Care Power of Attorney assigns a person, known as a health care agent or proxy, the authority to make healthcare decisions on your behalf if you are unable to do so.
Do Not Resuscitate (DNR) Order
A Do Not Resuscitate order instructs healthcare providers not to perform CPR or other life-saving interventions if a patient's breathing stops or if the patient's heart stops beating.
POLST or MOLST Form
