Assessment of Motor and Process Skills (AMPS)
Purpose: To measure a person's performance capacity for activities of daily living and instrumental activities of daily living. Population: Individuals of all ages with a variety of impairments.
Vineland Adaptive Behavior Scales (VABS)
Purpose: To assess personal, social, and adaptive functioning of individuals from birth to adulthood. Population: All ages from infancy through adulthood.
Functional Independence Measure (FIM)
Purpose: To assess the level of a patient's physical and cognitive disability and to track the progress over time. Population: Adults with disabilities requiring rehabilitation.
COPM (Cognitive Orientation to daily Occupational Performance)
Purpose: To enable clients to identify and prioritize everyday issues that restrict their occupational performance. Population: Individuals with cognitive disabilities.
The Activity Card Sort (ACS)
Purpose: To identify a person's level and extent of participation in instrumental, leisure, and social activities. Population: Primarily with older adults, but can be used with a variety of age groups.
Kohlman Evaluation of Living Skills (KELS)
Purpose: To determine an individual’s knowledge and/or performance of 17 basic living skills needed to live independently in five main areas. Population: Adolescents and adults in acute psychiatric clinics, as well as elderly populations.
Pediatric Evaluation of Disability Inventory (PEDI)
Purpose: To assess capabilities and performance in children, including aspects of self-care, mobility, and social function. Population: Children aged 6 months to 7 years.
Canadian Occupational Performance Measure (COPM)
Purpose: To detect changes in a client's self-perception of occupational performance over time. Population: Individuals of all ages.
Fugl-Meyer Assessment (FMA)
Purpose: To evaluate motor functioning, balance, sensation, and joint function in individuals with post-stroke hemiplegia. Population: Stroke patients.
Occupational Circumstances Assessment Interview and Rating Scale (OCAIRS)
Purpose: To assess aspects of occupational adaptation and occupational identity. Population: Individuals with physical or psychological impairments.
Rankin Scale
Purpose: To measure the degree of disability or dependence in the daily activities of people who have suffered a stroke. Population: Stroke survivors.
The Bayley Scales of Infant and Toddler Development
Purpose: To assess developmental functioning in infants and toddlers. Population: Young children, typically from 1 to 42 months old.
Beck Depression Inventory (BDI)
Purpose: To measure the severity of depression. Population: Adults and adolescents.
Mini-Mental State Examination (MMSE)
Purpose: To screen for cognitive impairment and to monitor changes in cognitive functioning over time. Population: Primarily used with the elderly, especially those suspected of having dementia or Alzheimer's disease.
Glasgow Coma Scale (GCS)
Purpose: To assess consciousness in trauma patients and predict the likelihood of a patient attaining independence post-injury. Population: Trauma patients.
Peabody Developmental Motor Scales (PDMS-2)
Purpose: To assess the motor skills of children. It evaluates both fine and gross motor functions. Population: Children, generally between the ages of birth to 5 years.
Rancho Levels of Cognitive Functioning
Purpose: To describe the cognitive and behavioral patterns found in brain injury patients as they recover. Population: Individuals with brain injury.
Dunn's Sensory Profile
Purpose: To understand how sensory processing affects daily functioning. Population: Children and adolescents.
Sensory Integration and Praxis Tests (SIPT)
Purpose: To measure sensory integration processes that underlie learning and behavior. Population: Children, typically between 4 and 8 years old.
Bruininks-Oseretsky Test of Motor Proficiency (BOT-2)
Purpose: To assess the motor proficiency of children and young adults. Population: Children and young adults ages 4 - 21.
Barthel Index
Purpose: To assess the degree of independence in basic ADLs and mobility. Population: Primarily used with adults and the elderly, especially those who have physical disabilities.
Rivermead Mobility Index (RMI)
Purpose: To measure gross functional mobility of patients who have sustained trauma or stroke. Population: Adults who have experienced trauma or stroke.
Short Sensory Profile (SSP)
Purpose: To measure a child's sensory processing abilities and to identify sensory processing difficulties. Population: Children typically aged 3-10 years.
ECHOS (Early Childhood Home Observation for Measurement of the Environment)
Purpose: To rate the quality and quantity of stimulation and support available to a child in the home environment. Population: Infants and children up to age 6.
Berg Balance Scale
Purpose: To measure balance among older adults by assessing performance in 14 different tasks. Population: Often used with elderly to assess fall risk.
Allen Cognitive Level Screen (ACLS)
Purpose: To evaluate individuals' cognitive function relative to cognitive disabilities and provide a baseline for cognitive levels. Population: Individuals with cognitive dysfunction.
Model of Human Occupation Screening Tool (MOHOST)
Purpose: To gather information about an individual's occupational functioning. Population: Diverse range, including mental health and physical disabilities.
Functional Reach Test
Purpose: To assess stability by measuring the maximum distance one can reach forward while standing in a fixed position. Population: Primarily used with older adults; applicable to various populations with balance concerns.
Lawton IADL Scale
Purpose: To assess an individual's ability to perform instrumental activities of daily living independently. Population: Mainly older adults; can be used with any adult population.
