Problem Identification
Defining the issues that the patient is experiencing that OT can address. Examples include difficulty with self-care, decreased mobility, or cognitive impairments.
Intervention Planning
Developing a structured approach to therapy that will help the patient achieve their goals. Examples include exercise programs, cognitive retraining activities, or modifications to the home or work environment.
Discharge Planning
Preparing for the patient's transition out of OT services, which may include creating maintenance programs or referrals to other services. Examples include home exercise programs or connection to community resources.
Client Education
Teaching patients about their conditions, how to manage them, and how to prevent complications. Examples include instructing on ergonomic body mechanics, energy conservation techniques, or use of adaptive equipment.
Implementation
The actual carrying out of the treatment plan through therapy sessions and interventions. Examples include guided exercises, practicing daily living skills, or environmental modifications.
Assessment
Gathering information about the patient's condition, abilities, and environment to inform the treatment plan. Examples include interviews, observation, and standardized tests.
Cultural Competency
Providing care that respects the cultural, social, and spiritual values of the patient. Examples include respecting cultural beliefs in treatment approaches, adapting interventions to be culturally sensitive, or incorporating language services.
Assistive Technology
Using devices to assist in improving functional capacities. Examples include wheelchairs, communication devices, or adaptive utensils.
Documentation
Recording the treatment provided, patient's progress, and any changes made to the treatment plan. Examples include progress notes, therapy session summaries, and updated treatment plans.
Care Coordination
Collaborating with other healthcare providers to ensure that treatment is comprehensive and avoids duplication. Examples include interdisciplinary meetings, community liaisons, or care conferences.
Activity Analysis
Breaking down an activity into its component parts to identify where the patient may have difficulties and to determine potential modifications. Examples include analyzing cooking tasks, child care routines, or work-related activities.
Evidence-Based Practice
Integrating the best current research evidence with clinical expertise and patient preferences. Examples include using research studies to guide treatment methods, applying new therapy approaches, or revising protocols to reflect current standards.
Outcome Measurement
Assessing the results of the treatment in relation to the set goals. Examples include using standardized outcome measures, client self-report, or observation of changes in function.
Therapeutic Exercises
Prescribing and instructing specific exercises to improve function, strength, or mobility. Examples include range of motion exercises, strengthening routines, or balance and coordination activities.
Family and Caregiver Training
Educating family members or other caregivers on how to support the patient's treatment goals. Examples include teaching lifting techniques, strategies for behavior management, or how to implement a home program.
Advocacy
Speaking up for the rights and needs of the patient to facilitate access to necessary services. Examples include advocating for funding for equipment, improved accessibility in public buildings, or policy changes that benefit those with disabilities.
Environmental Modification
Adapting the physical environment to improve function and independence. Examples include installing grab bars, modifying workplace layout, or recommending specialized equipment.
Goal Setting
Creating specific, measurable, achievable, relevant, and time-bound (SMART) goals for the patient's treatment. Examples include improving grip strength, increasing independence in dressing, or optimizing workplace ergonomics.
Reassessment
Periodic evaluation to determine if the goals are being met and if the treatment plan should be adjusted. Examples include re-administration of standardized tests or conducting follow-up interviews.
Therapeutic Use of Self
Using one's personality, insights, perceptions, and judgments as part of the therapeutic process. Examples include displaying empathy, establishing boundaries, or using motivational interviewing techniques.
