Slip
An unintended action; a slip of action often occurs when the person's intention is correct but the execution goes awry. Example: Typing the wrong letter on a keyboard.
Lapse
A memory failure; it happens when a routine task is forgotten. Example: Forgetting to save a document before closing it.
Mistake
An error resulting from a faulty plan or decision. Examples: Choosing the wrong method to solve a problem, misdiagnosing a medical condition.
Rule-based mistake
Mistakes in applying good rules to the wrong situation or bad rules that lead to undesired outcomes. Example: Administering a standard medical procedure that's contraindicated for the patient.
Knowledge-based mistake
Errors made when facing a novel situation and the individual lacks the necessary knowledge. Example: An engineer incorrectly assessing an unknown machinery fault.
Skill-based error
Occur in highly practiced tasks when attention lapses, usually due to complacency or over-familiarity. Example: A skilled musician hitting a wrong note.
Perception error
An error made when sensory input is misperceived or misinterpreted. Example: A pilot misjudging the altitude of the aircraft on a hazy day.
Procedure violation
Intentionally not following established procedures or rules. Examples: Skipping steps in safety protocol, ignoring traffic signals.
Memory lapse
Forgetting to carry out an action or part of an action. Example: A server forgetting to place a customer's order.
Attention failure
Failing to pay attention to the relevant information. Example: Not noticing a stop sign and driving through it.
Loss of situational awareness
The failure to correctly understand what is happening in the environment around you. Example: A driver not noticing icy road conditions.
Interpretation error
Mistake made when one misinterprets or inaccurately assesses a situation. Example: Misreading a patient's symptoms and suspecting the wrong illness.
Decision error
A bad choice or judgment in response to a situation. Examples: Investing in a poor stock based on insufficient research, choosing an unsafe route.
Communication error
Failures during the exchange of information. Example: A nurse misinterpreting a doctor’s dosage instructions.
Violation of protocol
Deliberate disregard for rules, standards, or procedures. Examples: Unauthorized access to confidential files, deliberately ignoring cleaning protocols in a lab.
Action bias
Preference for action over inaction, which may lead to unnecessary or incorrect actions. Example: A football goalie diving in one direction during a penalty kick instead of staying in the center to increase chances of blocking.
Confirmation bias
Tendency to seek, interpret, and create information that confirms existing beliefs. Example: A doctor dismissing evidence because it contradicts their diagnosis.
Overconfidence bias
When a person's confidence in their judgment or abilities is greater than the accuracy of those judgments. Examples: A trader overestimating their financial forecast abilities, causing significant investment losses.
Availability heuristic
Over-relying on information that is readily available or recent when making decisions. Example: Judging the likelihood of a plane crash based on recent news stories rather than statistical data.
Omission error
Occurs when a required action is not performed. Example: An air traffic controller failing to issue a crucial command to a pilot.
