Balance Training
Exercises that challenge and improve stability. Note the types of balance exercises performed, level of difficulty, and the patient's ability to maintain balance.
AROM
Active Range of Motion - The range of movement a patient can perform unaided. Document the AROM in degrees and note any pain or limitations.
TherAct
Therapeutic Activities - Dynamic activities to improve functional performance. In patient notes, detail the activity, any adaptive techniques used, and the outcome or progress.
TherEx
Therapeutic Exercise - Exercises aimed at remediating impairments and functional limitations. In patient notes, detail exercises prescribed, sets, reps, resistance, and patient tolerance.
Assistive Devices
Tools that aid in mobility or activities of daily living. Document any assistive devices prescribed or used, their purpose, and instructions given to the patient.
Electrotherapy
The use of electrical energy for medical purposes. In notes, specify the electrotherapy type (e.g., TENS), parameters of application, duration, and the patient's response.
ADLs
Activities of Daily Living - Tasks that are essential for self-care. In patient notes, document the patient's ability or difficulty in performing ADLs.
Neuromuscular Re-ed
Neuromuscular Re-education - Techniques to improve movement, balance, coordination. Detail in patient notes the exercises or activities used, the patient's performance, and any progress made.
Transfer Training
Teaching patients how to move from one position to another safely and independently. Record in the notes the type of transfers practiced and the patient's ability to perform them.
Manual Therapy
Hands-on techniques to manipulate muscles and joints. In patient notes, describe the type(s) of manual therapy used and the patient's response, including changes in symptoms or function.
MMT
Manual Muscle Testing - A way to assess muscle strength. In patient notes, use a scale (0-5) to indicate muscle power.
Pain Scale
A numerical scale used to measure pain intensity. Document the patient's reported pain level using a scale like 0-10, where 0 is no pain and 10 is the worst pain imaginable.
Mobilization
Manual therapy techniques to improve joint movement. In patient notes, document specific techniques used, patient's response, and any changes in ROM or pain.
TherMod
Thermal Modalities - Application of heat or cold to treat symptoms. In patient notes, document the type of modality, area of application, duration, and patient response.
PROM
Passive Range of Motion - The amount of movement at a joint when the patient relaxes and the clinician moves the joint. Document PROM in degrees and compare it to the normative range.
Biomechanical Analysis
The study of body movement to determine dysfunction or inefficiencies. Document observations of the patient's biomechanics and any relevant corrective measures prescribed.
Modality
Any physical therapy treatment technique, such as heat, ice, ultrasound. In patient notes, specify the modality type, parameters, duration, and patient outcome.
Goniometry
A technique to measure joint angles. In patient notes, record the initial and final angles during movement using a goniometer to measure ROM.
ROM
Range of Motion - Refers to the extent of movement of a joint. In patient notes, express ROM quantitatively (degrees) or qualitatively (full, limited, etc).
HEP
Home Exercise Program - Tailored exercises for patients to perform at home. Describe the HEP's exercises, frequency, duration, and the patient's understanding in notes.
SOAP
Subjective, Objective, Assessment, Plan - a format for documenting healthcare encounters. In patient notes, record the patient's subjective experience, objective measurements, clinician's assessment, and the treatment plan.
AAROM
Active Assistive Range of Motion - The amount of joint motion performed by the patient with some assistance. In patient notes, describe the level of assistance required and movement achieved.
Outcome Measures
Standardized tests to quantify a patient's functionality and progress. Document the name of the measure used, the score, and implications for the treatment plan.
Functional Mobility
The ability of a patient to move around effectively in different environments. In patient notes, document specific functional tasks and the patient's performance/ease of completion.
PT
Physical Therapy - Professional care provided by a physical therapist. In documentation, refer to the type and duration of PT services provided, treatment goals, and patient progress.
