Intake Assessment
The process of gathering detailed information to understand a client's situation and needs. Example: A social worker conducts interviews to collect a client's history, current situation, and service needs.
Needs Identification
Analyzing information collected during intake to identify the client's needs and challenges. Example: Determining a client's need for housing, employment, or mental health services.
Service Planning
Creating a structured plan of action to address the identified needs of the client. Example: Developing a treatment plan that includes therapy sessions, vocational training, and budgeting assistance.
Advocacy
Acting on behalf of the client to secure services and resources they are entitled to. Example: A social worker contacts agencies to arrange for emergency housing and food stamps for a homeless client.
Monitoring
Regularly checking on the client's progress and the effectiveness of the service plan. Example: Scheduling monthly meetings to assess client's adherence to a medication regimen.
Evaluation
Assessing the outcomes of the case management process to determine the success of the interventions. Example: Comparing the client's current status to the initial assessment to measure improvement.
Reassessment
An ongoing process of evaluating the client's needs and adjusting the service plan accordingly. Example: Revising a client's service plan after they achieve stable employment to focus on education goals.
Case Closure
The process of formally ending the case management services once goals are met or services are no longer required. Example: Terminating case management after a client has been successfully integrated back into the community.
